Korean Society for Quality Improvement in Healthcare et al.: Understanding and Application of Patient Safety Reporting Learning
It's a patient safety system to have a device that shuts off people at the point where mistakes are done, even if they make mistakes. So instead of finding the cause in repeated human mistakes, we need to have a system that allows the blockage to work even if you make mistakes. Patient safety reports and learning systems are the foundation for building these systems. The looking and learning system is the beginning of patient safety. And while international studies and books vary, it's hard to find a bookbook that is based on real cases of experts in the country. I know the theory, but I need a real-life reference to patients who work at the actual medical center.
