Akira Ishibashi: Why do accidents repeat?
Seven of the safety Korean series. The fact of the accident, together with understanding the relationship, cannot be made to make effective measures without deep study of why we couldn't cope properly. The Great Globo subway fire investigation is a good example of the need to create the fastest and most clear antirepeat measures. But it doesn't matter if the thinking is based on detailed and rigorous technical analysis. We need to focus on extracting the problems of human behavior for recurrent control and finding effective ways to make each one work. The way the author introduces the Berry Agenda tree analysis is a clock-intensive way of monitoring human behavior around information processing, including human information recognition, judgment, decision making. In other words, the common understanding of the flow of accidents by field agents is also called roadmaps. In general, there is hardly any accident that occurs because of one single cause. A few of the things behind are chained up, and you can't cut them off, so you end up with an accident. It is therefore rash to think that a thorough improvement in one estimate would provide a complete recovery against the relapse. In other words, the accident should not only provide a measure of action for the person, but also a comprehensive approach across a wide range of areas, including machinery, environment and systems.
